ANNUAL SUPPORT PRICING CHANGE EFFECTIVE OCTOBER 1, 2007:

If you have not done so already, please complete the Support Authorization and Payment Form that must be completed and returned via fax in order to receive support services. Once this has been processed, a Toll-Free phone number will be used for all support calls. This number will also be used to request custom document services.

Support and custom document services will not be performed without a valid Support Authorization and Payment Form on file for your company.

Monthly Fee: $39/month

Network Installations: $39/month for first user, $15/month for each additional network license

Custom Reports (Crystal Version 8.0/8.5): $75/hr (one hour minimum, includes file installation on system)

Phone Training: $75/hr (by appointment only)
